Before you fill out this form, please note the following...

• This form is only to be filled out for those characters that cannot be used in any way. Please use the link above for any other issues with your character.
• If you have not entered a game with your character within the past 90 days, it may have expired. We do not restore expired or deleted characters. We cannot create characters or items to replace losses.
• To find your character names, go to the Save folder in the Diablo II directory. You should find a folder called USWest, USEast, Europe, and/or Asia. In these folders you can find the names of your characters on each server realm.
• Please submit a separate request for each character you are having problems with.
