4) Verification.
Due to legal and security concerns a valid government-issued document is required to verify your identity. This government-issued document must display your full legal name and date of birth.
Evidence of eligibility is also required to make changes to the accountholder's name. If you require changes to the accountholder name, or Parental Controls, you must supply documentation that verifies your identity as well as ID or documentation matching the person currently registered to the account.
